The normal function of articular cartilage is to protect bones and involves ensuring the sliding and absorption of forces during loading. Articular cartilage is constantly exposed to mechanical stresses, which are necessary for its health. The loss of the ability to respond to stresses and the metabolic changes caused by them leads to degeneration as seen in OA.
Cartilage consists of chondrocytes, intercellular substance, collagen fibers, and proteoglycans.
Synovial fluid is a yellowish viscous fluid that fills the joint cavity. It is a superfiltrate of plasma containing various substances and hyaluronic acid. Its main functions are to provide nutritional support and lubrication to cartilage and to absorb shocks. The shock-absorbing action of synovial fluid protects cartilage and bones from the stresses developed when we move, walk, run, etc.
Hyaluronic acid is produced by chondrocytes and synovial cells and is released into the joint cavity. It penetrates the surface layer of cartilage and synovial membrane creating a protective layer against mechanical injuries and chemical irritations.
In the case of osteoarthritis, the elasticity and viscosity of synovial fluid are significantly lower compared to a normal joint. A major cause is the reduction in the molecular weight and concentration of hyaluronic acid, resulting in decreased 'lubricating' and protective effects. When this decreases, the weight stresses from normal movement cause damage to the cartilage surface and synovial membrane.
