Atopic Dermatitis

Atopic Skin: What Happens to the Skin Barrier

Atopic dermatitis, also known as atopic eczema, is a chronic inflammatory skin condition with flare-ups and remissions. At its core is a skin barrier that does not properly retain moisture. The skin loses water, dries out, and at the same time allows irritants and allergens to penetrate more easily. This is where the cycle of itching and scratching begins, which in turn worsens the barrier.

Important clarification: atopic dermatitis is a medical condition. Diagnosis, assessment of severity, and any medication fall under the dermatologist. The products in this category are cosmetics and daily care dermatological products. Their role is supportive, i.e., to keep the skin moisturized and reduce the feeling of dryness and discomfort between flare-ups.

Emollients Are the Foundation of Daily Care

The emollient, i.e., the softening lotion or cream, is the product applied daily and to the entire surface of the body, even when the skin appears calm. Consistency matters more than price or brand. Dermatological recommendations speak of generous amounts, at least one to two times a day, and always within a few minutes after bathing, while the skin is still slightly damp.

The texture is chosen based on the severity of dryness. The lotion spreads easily over large areas, the balm and butter provide a more intense shielding effect on the most troubled areas, such as elbows, knees, wrists, and neck.

What to Look for in the Composition

• Ceramides, cholesterol, and fatty acids: replenish the lipids missing from the barrier.
• Glycerin: the most studied moisturizing ingredient, well tolerated even on very sensitive skin.
• Shea butter and emollient ingredients: limit transdermal water loss.
• Niacinamide and D-panthenol: improve skin comfort and feel.
• Prebiotics and thermal spring water: target the balance of the skin microbiome.
• Fragrance-free, alcohol-free, paraben-free: fewer ingredients means fewer chances of irritation.

The Bath Does Half the Job

Hot, long showers and common soaps remove lipids from an already insufficient skin. Prefer lukewarm water, duration of five to ten minutes, and mild cleansers without soap, the so-called syndets, or emollient bath oils. Dry with gentle pats rather than rubs, and immediately apply the emollient. See also the categories soaps and shampoos and body dry skin for mild cleansing options.

What Worsens Flare-Ups

The most common aggravating factors reported by patients are wool and synthetic fabrics in contact with the skin, sweat left on the skin, very dry winter air and air-conditioned room air, scented detergents and fabric softeners, dust, and stress. Cotton, washing clothes without fabric softener, cutting nails short, and maintaining a cool room temperature also help.

Popular Lines for Atopic Skin

In this category, lines designed specifically for atopic skin stand out. The La Roche-Posay Lipikar is one of the most sought-after, with options in both lotion and balm and a soap-free cleanser. The Avene XeraCalm and the URIAGE Xemose rely on thermal spring water and lipid replenishers. The Bioderma Atoderm covers the entire spectrum from daily hydration to intensive care, while the CeraVe with its ceramides and the Eucerin AtopiControl have a steady following. For topical relief in troubled areas, the restorative balms of the Cicaplast, Cicalfate and Cicabio types are widely used.

Frequently Asked Questions

How much emollient is really needed?

Much more than most people think. Dermatological guidelines speak of several hundred grams per month in an adult with extensive dryness. If a package lasts you months, you are probably applying a little.

Do I continue the emollient when the skin is clear?

Yes, and this is the most important point. Daily hydration during periods of calm is what keeps the barrier in better condition. Stopping as soon as the picture improves is the most common mistake.

Can I combine it with treatment prescribed by my doctor?

The emollient usually coexists with treatment, but the order and timing of application are determined by the treating physician. Ask your dermatologist or pharmacist.

Is it hereditary?

There is a strong genetic predisposition and often an individual or family history of allergic asthma or allergic rhinitis. It is not contagious.

When should I see a doctor?

When lesions extend, when itching disrupts sleep, when signs of infection appear such as yellowish crusts, pus, pain, or fever, and when the condition does not improve with daily care. In infants and young children, an assessment by a pediatrician or dermatologist is essential.
