The following factors represent the main causes of vitamin B12 deficiency:

- Alcohol and drug consumption. The breakdown of alcohol and drugs requires a lot of vitamin B12 and, more importantly, causes stress in the stomach and liver. This can lead to reduced production of the intrinsic factor and insufficient reabsorption in the intestine.
- Diet with foods that do not contain sufficient vitamin B12 (more common in vegetarians and vegans) – malnutrition.
- Increased demand for vitamin B12 caused by other factors such as heavy metals, toxins, medications, and injuries.
- Worms / parasites. Specifically, tapeworms consume large amounts of vitamin B12. There is not enough supply, while the worm remains in the body.
- (Partial) removal of the stomach as a result of surgery.
- Age. Often, the elderly develop a weaker stomach epithelium. A vitamin B12 deficiency is usually the reason for fatigue, memory loss, and depressive mood among the elderly.
- Medication. Drugs containing hormonal contraceptives containing estrogens and hormone supplements, diabetes medications (metformin), high blood pressure medications (ACE), heart rhythm medications (beta blockers, nitrate sprays, nitroglycerin), cholesterol-lowering drugs (statins), and impotence treatments.
- Liver disease (storage and transport problems).
